Submitting an electronic version of the Legacy Quilter Nomination Form

There are two places that the Legacy Quilter Nomination Form can be completed.  

1. mississippiquilt association.org

Upon completion of the form, the form is sent via email to mississippiquilt@gmail.com automatically.
You will follow up by emailing the photos of the quilter and quilt to mississippiquilt@gmail.com.

 2. mqa.clubexpress.com

You complete the online form at mqa.clubexpress.com / Forms / Legacy Electronic.
At the end of the form, you upload the photos.  After looking over the form, press Submit and your are done.
